Dagenhart Health And Wellness remains committed to in-network status with most major medical commercial insurance carriers, Medicare, and Maryland's Medical Assistance Program to help support and provide services to our community.

Many insurance providers include mental health services within their benefit plans. This helps make it more accessible and affordable to anyone seeking treatment.

  • Medicare (As of May 2024 we have stopped accepting any new Medicare patients. We apologize for any inconvenience)

  • Optum/United Behavioral Health /United Healthcare /UMR (All Providers)

FOR HMO PLANS PLEASE CONTACT YOUR INSURANCE COMPANY TO CONFIRM IF A PRE-AUTHORIZATION FOR SERVICES IS REQUIRED.

NOT ALL OF OUR PROVIDERS ARE IN-NETWORK WITH ALL INSURANCE COMPANIES LISTED ABOVE. Please contact our offices to confirm your provider is in network prior to your first appointment.

Fees/Payments

Fees for insurance networks are pre-contracted. The client/patient is responsible for all co-pays, co-insurance, and/or any fees not covered by insurance. Due to insurance limitations, some of our providers do not accept insurance reimbursement. However, we can provide you a statement to be submitted to your insurance company.

If we are a participating provider with your insurance plan or have an agreement with your insurance company, we will submit insurance claims for you.  Although our staff will verify your insurance benefits and coverage, you are responsible to know the details of your insurance policy for out-patient mental health, including deductibles, co-pays, co-insurances, pre-authorizations, physician referrals, and any other limits.  If we do not participate with your insurance plan, we will provide for you a statement that can be submitted to your insurance company by you.  It is your responsibility for any outstanding balance not covered by insurance and agree to pay any such amounts in a timely manner. 

We require patients to keep an active credit card on file to make the checkout process easier, faster, and more efficient. Periodically and upon your request you will receive statements from us. Please contact our billing department if you have any questions about your statements and/or account(s).

No Surprises Act

In compliance with the No Surprises Act, all patients have the right to receive a “Good Faith Estimate” explaining how much health care will cost.

Under the law, health care providers need to give patients who do not have insurance or who are not using insurance an estimate of the bill for health care items and services.

  • Patients have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

  • Patients should make sure their healthcare provider gives them a Good Faith Estimate in writing at least 1 business day before receiving healthcare services or items. Patients can also ask their health care provider, and any other provider they choose, for a Good Faith Estimate before scheduling an item or service.

  • If a patient receives a bill that is at least $400 more than their Good Faith Estimate, they can dispute the bill.

  • Patients should be sure to save a copy or take a picture of their Good Faith Estimate.
